• What are the benefits of MEMS Alert Membership?

MEMS Alert members are never billed for medically necessary emergency ambulance transportation.  MEMS will accept payment from your insurance provider as payment in full.  The MEMS Alert program will absorb any copays or deductibles associated with your emergency transportation.

MEMS Alert also provides membership benefits for non-emergency ambulance transportation and other services provided by MEMS.  As a member of the MEMS Alert program, you will not be billed for any medically necessary non-emergency transportation approved by your insurance provider.  For all services not covered by insurance or Medicare, MEMS Alert offers a 40% discount from normal service rates.

• What does MEMS Alert Cost?

The membership rates for MEMS Alert listed below are determined by the insurance coverage you already have.  And the price of a single membership will provide the benefits of MEMS Alert to you and to any immediate family members living in your home.  An immediate family member is a spouse, child, or parent.  So for one $60 - $80 membership, all immediate family members will enjoy the benefits of MEMS Alert.

When determining the price for MEMS Alert, be sure to select the highest rating for any individual in your family that would be covered by the membership.  For instance, if one family member was covered by two insurance policies (1st category) and a second family member had no insurance coverage (3rd category), the correct price would be $80.  The categories and membership rates are:

1. $60     Members covered by two (2) insurance policies.  This category would include individuals and family members covered by two commercial policies.  This category also also include Medicare Part B beneficiaries  with supplemental coverage such as Medipak, Medigap, or any other supplementary insurance.
2. $70     Members covered by only one (1) insurance policy.  Medicare Part B beneficiaries without any supplemental coverage will use this rate.
3. $80     Members without insurance coverage.  If you do not have health insurance coverage and you do not qualify as a Medicare Part B beneficiary, this is your rate.

Note that in order for you and your family to be eligible for MEMS Alert benefits, you must provide all of the information requested on the enclosed application form for each family member to be covered under your subscription.  Also note that as a renewing member in future years, you are eligible for a $5 discount off the price of the MEMS Alert subscription.

• How do I sign up for MEMS Alert Membership?

An application form for MEMS Alert is available below.  Simply click on ‘Download a MEMS Alert Application’ at the bottom of this page.   Complete the application form, include a check, money order, or credit card information, and mail to MEMS Alert, P.O. Box 2452, Little Rock, AR  72203. 

Your membership becomes effective the day we receive your payment, and will be in force for 12 months from this date.  You will be mailed a renewal application two months preceding the expiration of your membership.  Return the renewal application to continue your membership without interruption.  Be sure to take advantage of the $5 discount on your renewal date.

• Who Might Have Limited Benefits From MEMS Alert?

There are some people for whom MEMS Alert may provide a reduced benefit:

1. Persons who are covered by Medicaid benefits.  Medicaid pays 100% of the cost associated with ambulance transportation.  Therefore, MEMS Alert membership benefits will be limited to non-transportation services.
2. Individuals whose health insurance pays for 100% of the cost associated with ambulance transportation.  MEMS Alert membership benefits will be limited to non-transportation services.  New members should check with their health insurance provider to determine the benefit of MEMS Alert membership.
